This page will help you print the correct forms to create your own On-Line Newsletter. If you are looking for details about the newsletter, Click Here to return to the previous page.

To assemble Web page kit, print the following pages, fill them out and return them to me.

One Family Tree Page for your organization.	Help with this page.
One Individual Page for each member.	Help with this page.
One Family Detail page for your organization.	Help with this page.
Return package and payment to me.	Payment details

The first page you should print out and complete is the Family Tree page. It will list the Grandparents, their adult offspring and spouses, and the grandchildren from each adult offspring. The main page will allow me to understand the pieces that make up your organization. You will need to fill out one Family Tree Page.

Print One Family Tree Page for each website you are ordering.

Individual Page

Each member of your organization will need an Individual Page. You may be able to supply all of the details for each member, or you may not be able to. You may even elect to enter only basic information (Name, birthday, one photo, e-mail). This will allow each person to fill out their own personal information at their leisure.

Print one Individual Page for each member in your organization.

The last page you need to prepare is the Family Detail page. It will list the details of the organization that are common to everyone. These details include possible Web Site names (remember this will not include the last name of the family). You will also be offered the chance to list websites of interest to your family to be included in the website.

Print one Family Detail page for each website you are ordering.

Once these pages are completed, you are ready to send them to us to establish your site. Provide a check made out to Visible Web Solutions and mail the entire package to me.
